Here's the big bad I put together for the adventure I am working on "The Curse Of The Nevergale" With what I have done so far, I was locked into a theme of chains, so something with chains was needed to pull the story together.

Skhallic is guardian of the pit on the plane of steel,
and is sent to the earthly plane every six years to harvest
fresh souls to torment for his master Sith. He will take the form of
a bald naked muscular tattooed man who's face is bound in chains, and body is partly
wrapped in chains, the strange small interwoven black demonic runes that appear on Skhallic's
skin are in fact the tally of souls collected, and when the body is fully covered Skhallic will
return to the plane of steel.

Skhallic's chains will act as chainmail armour, and some are attached
to a variety of sharp implements and padlocks, that are used as deadly weapons as
they whip through the air at high speed.

Skhallic has the power to animate non living things, and will usually
be accompanied by some form of Living Statue to act as his guardian, the
bodies of the souls harvested will remain as undead under the control of
Skhallic until he returns to the plane of steel.

When his earthly form is destroyed Skhallic will return to the plane
of steel with the souls he has harvested, and on his next return he will
try to hunt down and harvest the souls of those who have vanquished him in the past.
